Form Descriptions

Individual Credit Application

Use the Individual Credit Application form to apply for credit for participation in workshops and conferences not offering approved USOE or District credit. Submission for credit must be made within two years of completing the conference or workshop.

Professional Development Application

Use this form to apply for credit, license points, and/or district professional development funding, for courses, workshops, or other professional development work provided by the district, school or department.

Master's Lane Equivalency Program

This is a Master's Lane Equivalency, not a Master's Degree. The only purpose of this program is to allow you to progress to the Master's salary lane. Teachers completing their Master's Lane Equivalency after December 1, 2011 will be unable to move to the Master's +20 or Master's +40 lanes from the equivalency. The equivalency is not transferable between districts, nor will it be accepted if you apply for a position within our district that requires a master's degree. To begin the program you must:

  • Be on the B+40 lane
  • BEFORE CLASS BEGINS submit a Master's Lane Equivalency application to Professional Development Services
  • BEFORE CLASS BEGINS receive confirmation that course work has been approved by the Master's Equivalency Committee.

Course Roster of Participants

This form is used as a roll. Upon completion of a course this list should be sumbitted to Professional Development Services with signature of the Authorized monitor verifying the individuals' completion of the course requirements.

Office Professional Incentive Program (OPIP) Course/Workshop Funding Application (Pre-Approval)

Funds are available for all employees paid on the SLEOPA Salary Schedule to take courses/workshops addressing topics and skills that relate to their current job position.

Office Professional Incentive Program (OPIP) Credit Application

Salt Lake City School District contract Office Professionals may receive credit towards a level change by attending courses/workshops addressing topics and skills that relate to their current job position.

Office Professional Incentive Program (OPIP) Level Change Application

Salt Lake City School District contract Office Professionals use this form to apply for OPIP level change.
