Employee Assistance Program

Salt Lake City School District provides employees an Employee Assistance Program (EAP) through Blomquist Hale for mental health services.  The EAP program is free to employees as long as the services are provided by Blomquist Hale.  If there is a need to be referred to mental health providers outside of Blomquist Hale's staff, employees will need to be enrolled in a District medical plan and Blomquist Hale will need to pre-authorize services with that plan and the employee will need to pay co-pays according to their plan summary.
