Transition and Adult Services Salt Lake City School District

Transition is an ongoing process that will support your student as they move from the K-12 school system to adult life.

Transition services are required for eligible students ages 16-21 who receive special education services by federal law, state law, and school district policy. IEP’s (Individual Education Programs), will reflect the students’ skills and needs in order to prepare them for life after they graduate, or “age-out”.

Salt Lake City School District offers two programs for students ages 18-22 with disabilities. VIP (Vocational Independence Program), and SCORE (Self-Advocacy, Community, Occupation, Recreation, and Education). Each program is designed to meet the individual needs of students who have disabilities in the Salt Lake City School District.

SCORE Program Overview:

SCORE strives to prepare students for the transition from school to adult life with meaningful experiences that prepare them to be successful in their communities and employment.

SCORE is designed to meet the individual needs of students, ages 18-22, who have disabilities in the Salt Lake City School District.  Our goal is to support family and students beyond school including, vocational training, daily living skills, leisure, self-advocacy, communication, and community relationships. Student choice and self-advocacy are critical elements to successful transition.  When your student leaves SCORE they will have obtained employment and/or a plan for employment, and community activities, that they can participate in during their adult lives.

Community Agencies and partners you will be introduced to through SCORE include DSPD (Division of Services for People with Disabilities), VR (Vocational Rehabilitation), CRP’s (Community Rehabilitation Program), The Parent Center, NAC (National Ability Center), Community Rec, Tanner Dance, LEAD and Elevate through the University of Utah.
