COVID Vaccination Clinics at Rose Park and Glendale
UPDATE: All of the available spots are filled, and the registration form is now closed. We are hoping to add more and will let our community know if we do.
To increase access and remove barriers for our community members receiving the COVID-19 vaccine, the Salt Lake Education Foundation and Salt Lake City School District have partnered with the county health department to hold vaccine clinics at the Rose Park and Glendale Community Learning Centers. Any community member who meets the current Utah Department of Health guidelines for receiving a vaccine (at least 50 years and older or 18 years and older with a qualifying pre-existing health condition) can receive a vaccine.
All participants must register in advance. You must also bring a form of identification (i.e., photo ID) and your insurance card if you have it (you are not required to have insurance).
