Notice of Cobra Benefits Rights

Purpose of this notice:  If you are enrolled, or soon will be enrolled, in group health insurance benefits under Salt Lake City School District' group health plan, this coverage entitles you to rights under the Consolidated Omnibus Budget Reconciliation Act of 1985, which is a federal law known as COBRA.  COBRA guarantees you the right to continue health coverage that would otherwise be lost due to the occurrence of certain events.  The purpose of this notice is to inform you of your rights and obligations under COBRA if you experience a qualifying event.
