District nurses are offering flu shots to students, siblings, and parents at your student’s school.If you are interested in receiving a flu shot at your student’s school clinic, please follow the instructions below to complete the appropiate Encounter Form.

Providing opportunities for students to get flu shots is an important part of every school year. District nurses are offering flu shots to students, siblings, and parents at your student’s school. If you are interested in reciving flu shots at your student school’s please see the instructions below, and complete the approprate form. For questions about flu clinic dates and times, please contact your school secretary.
If your family has one of the following insurance plans:
- Aetna
- United Health Care
- PHEP
- Cigna
- SelectHealth
- Blue Cross Blue Shield
- EMI
Complete both pages of the School Encounter. Please review the insurance requirements carefully.
- Make a copy of the front and back of the insurance card.
- Return all the paperwork to the school secretary.
If your student’s is uninsured, or has CHIP, Medicaid or is Native American/Alaska Native, complete the school encounter form. No insurance card necessary.
- Complete both pages of the School Encounter Form.
