Student's who currently have exemptions to immunizations or are requesting a new exemption. Will be required to provide a new exemption going into kindergarten and 7th grade. These exemption forms can be obtained by completing the immunization module at Immunization Education Module or by visiting your local health department. The school will not be able to accept your student’s previous exemption form. The new exemption claimed at 7th grade will be valid through 12th grade.

A parent may claim an exemption to immunization for medical, religious, or personal reasons, as allowed by Section 53G-9-303 of the Utah Statutory Code.
