Teacher PD Forms and Transcripts

As of July 2026, the Utah State Board of Education (USBE) discontinued the practice of allowing districts to grant individual credit. Districts can no longer submit courses on behalf of individuals for USBE or District Credit. Credit may only be awarded for courses offered through SLCSD's Kickup catalog or USBE's Altitude catalog. However, USBE Credit and credit earned through an accredited university may still be submitted for lane change consideration directly to Human Resources.
